Publication Format for Psychwatch NZ.

Please find below a guide to presenting your story for publication on Psychwatch NZ. All stories submitted for publication must have a written informed consent from the service consumer, and / or the nominated representative thereof. The identity of the service consumer shall at all times remain anonymous on Psychwatch NZ:

Section 1 – Introduction: (What happened? Who was involved in what happened? Who was the service provider? What staff of the service provider were involved in your case?)

Section 2 – Complaints & Redress Process: (What steps were taken to register formal complaints, to who, what responses did you get, and what was the end result?)

Section 3 – Evidence: (What hard copy evidence are you able to provide Psychwatch NZ to ensure that you can back up the claims you are making?)

Section 4 – Conclusion: (What is the outcome you would like for your case?)

Terms & Conditions:

1/ Every author of any story submitted for publication on Psychwatch NZ must have first secured informed consent (and evidence of informed consent) from the mental health consumer and / or family of the mental health consumer making the written submission to Psychwatch NZ, and agrees to the identity of the mental health consumer remaining anonymous on Psychwatch NZ.

2/ Every author of any story submitted for publication on Psychwatch NZ must be able to provide evidence of the claims contained in the submission, prior to publication on Psychwatch NZ. Such evidence may include (but is not restricted to) copies of case notes, complaint letters, and other relevant correspondence relating to the submission.

3/ Every author of any story submitted for publication on Psychwatch NZ must agree to allow the nominated parties named in their submission to have the opportunity to respond to the claims being made through appropriate due process, and to invite the complainants into the relevant complaints process, prior to publication of their submission on Psychwatch NZ.

4/ Every author of any story submitted for publication on Psychwatch NZ must agree to have followed all relevant complaints processes available to them, prior to the publication of their submission on Psychwatch NZ, and to provide evidence of this process.

5/ Every author of any story submitted for publication on Psychwatch NZ must agree to indemnify Psychwatch NZ of any responsibility regarding any outcome whatsoever of publishing their submission on Psychwatch NZ.
